These Terms of Service govern Zito Media’s Video, Internet, Digital Voice, and Mobile Services. Please review them carefully, as they explain your rights, responsibilities, billing terms, service limitations, and important disclosures.
This Master Services Agreement (“Agreement”), together with the applicable Service Schedules,
Work Order, and any referenced policies and practices, constitutes the entire agreement between
Treasure Lake, L.P. and Zito Media, L.P. and each of their affiliates (collectively, “Zito Media,”
“we,” “us,” or “our”) and the account holder identified on the accompanying Work Order or
statement (“Subscriber,” “you,” or “your”).
This Agreement governs your receipt of any combination of the following services: Video Service,
Internet Service (HSD), Digital Voice Service, and Mobile Service (each a “Service” and
collectively, the “Services”). This Agreement supersedes all prior written or oral agreements
between the parties.
Your acceptance of any Service, whether by signing a Work Order, activating a Service, or
continuing to use a Service after receiving notice of changes to this Agreement, constitutes your
acceptance of the terms and conditions contained herein. The use of the Services by any person
other than you is also subject to this Agreement.
Zito Media has the right to add to, modify, or delete any term of this Agreement, including Service
Schedules and referenced policies, at any time. An online version of this Agreement will be
accessible at www.zitomedia.com. Zito Media will notify you of any significant changes.
Changes become effective immediately except where applicable law requires a notice period, in
which case the change becomes effective at the end of the requisite notice period.
Your continued use of the Services after the effective date of any change constitutes your consent
to the change. If you do not agree to any change, you must immediately stop using the affected
Services and notify Zito Media that you are terminating those Services. If you find any change
unacceptable, you have the right to cancel your service without penalty during the notice period.
Not all Services are available in all geographic areas. Zito Media reserves the right to determine
the availability of Services in specific areas. For current service availability, rates, and offerings,

You agree to pay Zito Media for all use of the Services, installation and applicable service charges,
equipment, device installment plans (if applicable), and all applicable local, state, and federal fees
and taxes. You will be billed monthly in advance for recurring monthly charges. Other charges will
be billed in the next practicable monthly billing cycle following use or as otherwise specified.
Zito Media may change both the fees and the types of charges for the Services.
Zito Media will mail or electronically deliver a statement to you monthly, including an itemization
of charges for all subscribed Services. Statements require payment within twenty (20) days after
the bill date. Customers receiving a paper billing statement are charged a fee of $1.50 per month
to cover the cost of production and delivery. Customers may elect to receive an electronic version
at no charge.
Accounts with a delinquent balance will be assessed a late fee of up to $7.00. Late fees will not
exceed the maximum permitted by applicable law. Zito Media reserves the right to change the
late fee amount.
Zito Media offers several convenient methods to pay your monthly invoice, including payment by
mail, payment office or drop box location (where applicable), payment by telephone with Visa,
MasterCard, or ACH, automatic payment via bank account, Visa, or MasterCard, and online
payment at www.zitomedia.net. Agent-assisted payments may incur an additional processing charge.
A fee of $30.00 will be charged for each declined payment transaction. Fees will also be charged
for returned checks and chargebacks, as listed in the current price list.
If you subscribe to a bundle of two or more Services at a promotional or discounted rate, the
bundle pricing is contingent upon your continued subscription to all Services included in the
bundle. If you remove any Service from the bundle, the remaining Services will revert to their
then-current standard individual rates. Early termination fees, if applicable, will apply to the
bundle as described in the relevant promotional terms.
If you participate in a promotional offer with a discount on monthly service fees, your Services
will revert to the standard monthly fee at the end of the promotional period unless you terminate
the Service. If the promotional offer requires a minimum time commitment and you terminate
early, you agree to pay the applicable early termination fee as described in the promotional terms.
Zito Media may verify your credit standing with credit reporting agencies and may require a
security deposit, bank or credit card authorization as a condition of providing or continuing to
provide Services. Zito Media may deduct amounts from your security deposit or bill any payment
method on file for any past-due amounts, including for damaged or unreturned equipment.
It is your responsibility to report billing errors within thirty (30) days from receipt of the bill.
Errors not reported within thirty (30) days are waived.
All equipment provided by Zito Media (“Zito Media Equipment”) remains the sole and exclusive
property of Zito Media. You do not acquire ownership of any Zito Media Equipment by virtue of
payments made under this Agreement or by attachment to your premises. Upon termination of
any Service, you must arrange for the return of all associated Zito Media Equipment in the same
condition as received, excepting ordinary wear and tear.
You agree to provide Zito Media and its authorized agents access to your premises during regular
business hours upon reasonable notice to install, inspect, maintain, repair, replace, or remove
Zito Media Equipment, install software, conduct service audits, or check for signal leakage.
If you are not the owner of the premises, you warrant that you have obtained the owner’s consent
for such access.
Under normal operating conditions, installations within 150 feet of the existing distribution system
are performed within seven (7) business days after an order is placed. Appointment windows will
be either a specific time or a maximum four-hour block during normal business hours.
Zito Media will not cancel an appointment after close of business on the business day prior to the
scheduled appointment. If a technician is running late, Zito Media will contact you to reschedule.
For Services where you provide your own device, including but not limited to mobile handsets,
streaming devices, modems, or routers, you are solely responsible for ensuring the device meets
Zito Media’s published compatibility requirements. Zito Media is not responsible for the
performance, maintenance, or repair of subscriber-owned devices.
You agree to pay Zito Media the replacement cost, as specified in the then-current price list, for any
Zito Media Equipment that is not returned, is damaged beyond ordinary wear and tear, or has been
tampered with. Zito Media may charge a continuing monthly fee until outstanding equipment is
returned, collected, or paid for.
The Services are residential services offered for reasonable personal, non-commercial use only.
You will not resell, redistribute, or permit others to use the Services for commercial or enterprise
purposes. You agree to use the Services for lawful purposes only and in accordance with this
Agreement, the Acceptable Use Policy, and all applicable laws.
Zito Media reserves the right to suspend or terminate Services without prior notification if it
determines you have violated this Agreement. You are responsible for protecting the confidentiality
of your screen names, passwords, PINs, and any other security credentials associated with the
Services. Zito Media is entitled to assume that any use of the Services from your account or
premises is authorized by you.
Upon request made within thirty (30) days, Zito Media will issue credit for system outages not
resulting entirely from circumstances beyond Zito Media’s reasonable control that exceed one
service day in duration. The credit amount is determined based on your monthly services, the
number of services affected, and the total outage time. Credits are issued no later than the next
billing cycle following a determination that credit is warranted. In no event shall credits exceed
the applicable monthly service fees.
Zito Media shall have no liability for interruption of Services due to circumstances beyond its
reasonable control, including acts of God, flood, natural disaster, vandalism, terrorism,
government regulation, fire, civil disturbance, electrical power outage, computer viruses,
labor disputes, or severe weather.
Zito Media may conduct scheduled and unscheduled maintenance that may result in temporary
interruptions of Services. Where practicable, Zito Media will provide advance notice of
scheduled maintenance.
Either Zito Media or you may terminate all or any portion of the Services at any time. If you wish
to terminate, you must notify Zito Media by phone or mail to schedule a disconnect appointment
and provide access for equipment recovery. You are responsible for all charges until the date of
disconnection.
Customers who have pre-paid for service are entitled to a refund of any whole months of service
paid for but not used, less any amounts owed to Zito Media. The remaining balance will only be
refunded if it is at least equal to one month of service fees. Partial months and installation fees are
non-refundable.
Zito Media may disconnect your service for non-payment after providing written notice of its intent
to disconnect. Before restoring service, Zito Media may require payment of the full balance owed,
a security deposit, the published reconnection charge, and any applicable fees and taxes.
Early cancellation of Services provided under a promotional or term agreement will result in an
early termination fee as specified in the applicable promotional terms. For the current 2-year service
agreement discount promotion, the early termination fee is $20.00 per month for each remaining
month in the term.
Termination of Services does not relieve you of the obligation to pay all amounts owed to
Zito Media, return all Zito Media Equipment, and comply with the surviving provisions of this
Agreement.
Zito Media has no liability for the completeness, accuracy, or truth of programs, content, or
information transmitted through the Services. Zito Media is not responsible for any third-party
content, merchandise, products, or services made available through the Services.
To the maximum extent permitted by applicable law, Zito Media’s total liability for any claims
arising out of or related to the Services shall not exceed the total amount of service fees paid by
you during the three (3) months preceding the event giving rise to the claim.

via Facebook at facebook.com/myzitomedia, or writing to Zito Media, P.O. Box 665,
Coudersport, PA 16915.
Investigative action will be initiated on the same day if possible, but no later than the following
business day. Written complaints will receive a written reply within thirty (30) working days.
If you are dissatisfied with our handling of a complaint, you may also contact the local franchising
authority at the address printed on the back of your billing statement.
By providing comments, reviews, photographs, images, or other feedback on Zito Media’s website,
social media channels, or social media platforms related to Zito Media’s services, you grant
Zito Media an irrevocable, perpetual, worldwide, non-exclusive, royalty-free, sublicensable
license to reproduce, use, publish, display, modify, distribute, or otherwise use such content
for promotional and marketing purposes.
Zito Media will take reasonable efforts to ensure that such use does not disclose your personally
identifiable information without your consent. If you do not wish for your content to be used in this
manner, please notify Zito Media in writing. You are solely responsible for the content you submit
and represent that you have all necessary rights to do so.
This Agreement shall be governed by and construed in accordance with the laws of the
Commonwealth of Pennsylvania, without regard to its conflict of laws principles, and applicable
federal law. If any provision of this Agreement is found to be invalid or unenforceable, the
remaining provisions shall continue in full force and effect. Zito Media’s failure to enforce any
provision of this Agreement shall not constitute a waiver of that provision.
The following Service Schedules supplement the General Terms and Conditions in Part I. Each
Schedule applies only if you subscribe to the corresponding Service. In the event of a conflict
between a Service Schedule and the General Terms, the Service Schedule shall control with respect
to the applicable Service.
Zito Media offers various video services at rates and fees described at www.zitomedia.com.
Service tiers include Broadcast Basic Service, Basic Service, and Digital Basic Tier, which may vary
by geographic area. Current program offerings, channel lineups, and rates are available at
www.zitomedia.com or by calling 888-823-2154.
Subscribers may access video programming through Zito Media TV2Go using a compatible Roku
or Firestick device, or other supported devices as designated by Zito Media. The TV2Go
application includes an Interactive Program Guide and parental control features with PIN-based
restrictions.
In areas where Digital Basic Tier is available, you must subscribe to Basic Service with TV2Go in
order to receive the Digital Basic Tier. For availability, contact 888-823-2154 or visit

Zito Media has no liability for the content, accuracy, or availability of any programming.
Programming lineups and availability are subject to change. Zito Media may be required by
federal law to block certain scheduled programs, typically sporting events, to protect exclusive
broadcast rights.
Video Services are provided for residential, non-commercial use only. You may not redistribute,
retransmit, or provide access to Video Services to persons outside the premises identified in your
Work Order. Programs and materials received as part of the Video Service remain part of the Video
Service even if recorded or captured on a storage device.
You may order additional video services or change your service level by calling 888-823-2154.
A downgrade fee of $25.00 will apply to partial service removal. You have the right to rescind an
order for new services without charge prior to installation or activation. If a rate or charge
adjustment is scheduled and you request cancellation or reduction of service within thirty (30)
days of notification, your liability for the new rate ceases from the moment the adjustment
becomes effective.
Zito Media offers Internet service on a tiered basis. Each tier has throughput limits and
consumption limits. Zito Media may change these limits by amending the price list or this
Agreement. Current tiers, data caps, and pricing are available at www.zitomedia.com.
The following monthly data consumption limits apply: High Speed Internet — 250 GB; Super Speed
Internet — 500 GB; Ultra Speed Internet and Mega Speed Internet — Unlimited. Zito Media may
use technical means, including speed reduction, to ensure compliance with these limits and may
move you to a higher tier, with higher monthly charges, if your usage exceeds applicable limits.
Zito Media reserves the right to monitor usage patterns to facilitate provision of the HSD Service
and ensure compliance with this Agreement. Zito Media may employ reasonable network
management practices, including traffic shaping and prioritization, consistent with applicable law
and FCC regulations. Details of Zito Media’s network management practices are published at
www.zitomedia.com.
Your use of the HSD Service is subject to the Zito Media Acceptable Use Policy, published
separately and incorporated herein by reference. You may not use the HSD Service for operation
as an Internet service provider, for unauthorized hosting of websites, or for any commercial or
enterprise purpose. You must secure any wireless network within your residence by establishing
and using a secure password.
The HSD Service provides a connection to the Internet that may be unfiltered. Zito Media does not
control and assumes no responsibility for third-party content. While parental control features may
be available, such features may not be entirely effective and you or members of your household
may be exposed to unfiltered content.
Each device used with the HSD Service must meet minimum hardware and software requirements
as specified by Zito Media, which may change from time to time.
Material posted or transmitted through the HSD Service may be copied, republished, or distributed
by third parties, and Zito Media will not be responsible for any harm resulting from such actions.
You grant Zito Media a non-exclusive, worldwide, royalty-free, perpetual license to use material
posted on public areas of the HSD Service via your account for purposes consistent with operation
and promotion of the HSD Service. Unsolicited email (“spam”) is prohibited and Zito Media may
establish volume limits on outbound email.
Zito Media provides Digital Voice Service over its cable network using a voice-enabled cable
modem. This Service is dependent on electrical power and an active broadband cable connection.
You acknowledge that the Digital Voice Service, including the ability to access 911 emergency
services, may not operate during an electrical power outage or if your broadband cable connection
is disrupted. Any battery backup included in the voice-enabled cable modem may provide limited
backup service or none at all, depending on circumstances. In the event of a system-wide power
loss, battery backup will not provide service. Zito Media will not be responsible for losses or
damages arising from the unavailability of Digital Voice Service, including the inability to reach
911 or other emergency services.
The location associated with your Digital Voice Service is the address on your Work Order. You are
not permitted to move the voice-enabled cable modem to a different address. If moved, 911 calls
will appear to originate from the Work Order address, not the actual location.
The existing voice wiring inside your home may not support both Digital Voice Service and digital
subscriber line (DSL) service simultaneously. You may need to maintain separate wiring for DSL
service. The Digital Voice Service may not be compatible with certain data transmission services,
including fax and dial-up Internet access.
Zito Media does not guarantee that the Digital Voice Service will operate with home security or
medical monitoring systems. You must contact your security or monitoring provider to test
compatibility. You are responsible for any costs associated with such testing or configuration.
Zito Media offers Mobile Service, which provides wireless voice, text messaging, and mobile data
service through a wholesale arrangement with an underlying wireless network operator.
Mobile Service is subject to the coverage, capacity, and technical limitations of the network
partner’s infrastructure. Zito Media does not own or operate the underlying wireless network.
Coverage maps and service availability information are provided as general estimates and do not
guarantee service at any specific location. Actual coverage may vary based on terrain, buildings,
weather, network congestion, device capability, and other factors. Mobile Service may be
temporarily unavailable or limited in certain areas, including inside buildings, in tunnels, and in
remote or rural locations. Zito Media makes no warranty that Mobile Service will be available at
all times or in all locations.
If Zito Media provides you with a mobile device, such device remains subject to the terms of
Section 6 of the General Terms unless sold to you outright. Devices sold to you under an
installment plan are governed by the separate Device Installment Agreement.
You may use a compatible device that you already own. Zito Media will publish a list of
compatible devices at www.zitomedia.com. Zito Media is not responsible for the performance,
functionality, or repair of subscriber-owned devices and makes no warranty regarding their
compatibility with the Mobile Service.
A SIM card or eSIM profile is required for Mobile Service. SIM cards provided by Zito Media remain
Zito Media’s property and must be returned upon termination of Mobile Service. You may not
transfer, alter, or tamper with a SIM card or eSIM profile without Zito Media’s authorization.
Mobile data is subject to the usage limits, throughput limits, and network management practices
of your selected plan. Data usage is measured by the network partner’s systems and that
measurement is determinative for billing purposes. Once you reach your plan’s data threshold,
your data speeds may be reduced for the remainder of the billing cycle. Specific plan details,
including data allowances, throttling thresholds, and overage charges, if any, are available at
www.zitomedia.com.
Tethering and mobile hotspot use may be included in certain plans or available as an add-on
feature. If your plan includes tethering, it is subject to a separate data allowance as specified in
your plan details. Use of tethering or hotspot features for enterprise, commercial, or resale
purposes is prohibited.
Your Mobile Service may include domestic roaming on partner networks. Service quality and data
speeds may differ while roaming. Zito Media reserves the right to limit or restrict roaming if your
usage indicates that a disproportionate amount of your use occurs while roaming off the network
partner’s home network.
International roaming is not included in standard plans and must be activated separately.
International roaming rates, which may be substantial, are published at www.zitomedia.com.
You are solely responsible for all international roaming charges incurred on your account.
Zito Media recommends disabling data roaming on your device when traveling internationally
unless you have activated an international plan.
You may port an existing wireless number to Zito Media Mobile Service, subject to technical
feasibility and the cooperation of your current carrier. You may also port your Zito Media number
to another carrier upon termination of Mobile Service. Number porting does not automatically
terminate your account; you must separately request disconnection. During the porting process,
there may be a temporary interruption of service.
Mobile 911 service functions differently from traditional landline 911 service. Your location is
transmitted to 911 dispatchers using GPS, cell tower triangulation, or other methods, which may
not always provide precise location information, particularly indoors, in underground locations,
or in areas with limited network coverage. Zito Media makes no warranty regarding the accuracy
of location information transmitted during a 911 call. Mobile Service will not function if the device
has no battery power, is turned off, or is outside the coverage area.
You must immediately report a lost or stolen device to Zito Media by calling 888-823-2154.
Zito Media will suspend service to the device upon notification. You are responsible for all charges
incurred on your account prior to Zito Media’s receipt of your report. Zito Media is not liable for
unauthorized use of a lost or stolen device.
Upon termination of Mobile Service, you must return any Zito Media-provided SIM cards.
Devices purchased outright remain your property. Devices purchased under a Device Installment
Agreement remain subject to the terms of that agreement, including any acceleration of remaining
payments upon early termination. If your Mobile Service is part of a bundle, termination of the
Mobile Service may affect your bundle pricing as described in Section 5.6 of the General Terms.
Certain terms required by the network partner may apply to your use of Mobile Service. These
terms, if any, will be provided to you at the time of enrollment or activation and are incorporated
herein by reference. In the event of a conflict between the network partner’s pass-through terms
and this Agreement, the more restrictive provision shall apply.
